Keith Frazier is a Licensed Clinical Social Worker who has worked in a variety of agencies in several states in his forty-year career. He is a graduate of The University of Kentucky College of Social Work receiving his master’s degree in 1989. Prior to graduate school Keith worked in community mental health and substance abuse centers. Since he received his master’s he has worked in Indiana, Illinois, Ohio, and Kentucky and has held the highest degree of licensure as a social worker in all of these states. Currently he is licensed at the highest level as a clinical social worker in Kentucky, Ohio, and New Hampshire. Keith was previously licensed as a Chemical Dependency Counselor in Indiana and Kentucky. Since 2004 he has been in a private practice in his hometown of Ashland. In his practice Keith has offered therapy and continuing education for social workers. Since the pandemic these services are no longer offered . Keith is now willing to help people find appropriate services.
Keith provides quality assurance and clinical supervision services and to two agencies licensed in Ohio and one agency concurrently licensed in Ohio and New Hampshire.
